Hi Everyone,

Last week we covered how to read traces and create test cases from them. But what happens when you have an agent with many steps, tool calls, and branching paths? Reviewing individual traces doesn’t reveal systemic patterns.

This is where a Transition Failure Matrix helps.

To create one, you need to aggregate your failures into a grid that shows where errors cluster. The rows represent the last successful state (“From State”) and the columns represent where the first failure occurred (“To State”). Each cell counts how many times that transition led to a failure.

For example, in a text-to-SQL agent, you might see that GenSQL → ExecSQL transitions cause 12 failures, while DecideTool → PlanCal causes only 2. Now you know where to focus your debugging.

To build one:

Define states. List the steps your agent can be in: Plan, Search, GenSQL, ExecSQL, etc. Each tool call or reasoning step can be a state. Be as granular as needed to isolate problems.
Create the matrix. Rows are “From” states, columns are “To” states.
Populate the cells. The simplest approach is counting failures: for each failed trace, identify the last successful state before the error and increment that cell. But the matrix is flexible. Depending on your situation, you might populate cells with failure rates, latency percentiles, token costs, or any metric that helps you understand where things go wrong. Count of failures is a concrete starting point, but treat this as a general analytical tool you can adapt.

One thing to note: a state with many failures isn’t necessarily the root cause. ExecSQL might fail frequently after LLM-generated queries but succeed reliably when the user specifies the query directly. The “From State” column helps you distinguish these cases.
